

This service enables older people who have been assessed by the local authority as having moderate or low needs to access information and advice regarding meeting their care needs. The service will work with you to ensure that once you have decided what servcies you want to use, you can access these in the way that suits you best.
The service is based on an initial meeting with you to establish a plan with you to describe your needs and how you want them to be met and any necessary further work to ensure your needs are met, for example benefits advice, access to care services, day care, befriending, support to manage your financial affairs, information and advice about what is available locally, plus representation and advocacy to make it all happen.
