As the community support I visit fifteen cleints in the Bexley area. Once I have assessed them I will do their banking for them, draw cash for their shopping, set up direct debits, pay bills, amke appointments and arrange transport. Set up home files and sort post. Working as an advocate I write letters for clients, completes financial assessments and liaise with other service providers. I help transfer clients to sheltered housing, residential or nursing care, attending rehab meetings when clients are ready to go home after hospital stays.
