- Full assessment of patient needs
-Skills matching carers to the care needs and personality of the patient
-A designated case manager throughout the life of the care package including trouble shooting and increased interagency collaboration.
-Provision of regular reviews and reassessments
-Provision of timely feedback of any changes to both Patients and Clients

Bridges Healthcare specialises in the provision of high dependency care, complex care, long term care and palliative care. Bridges cares for patients with complex care needs including trachea-ventilated patients, physical disabilities, learning disabilities, rehabilitation and physiotherapy. Bridges also offers community visits, 24hr care, respite care and night care. We offer a range of domestic service including cleaning, shopping and cooking.
